Overview of Drug-Drug Interactions

When two or more drugs are used together, the pharmacological result is not necessarily the sum of the effects obtainable from the drugs used individually in the same doses. This is because one drug may affect the action of another, and this is termed drug-drug interactions (DDI). There are many mechanisms of drug interactions. Some are chemical, some are biochemical, and some are physiological/pharmacological. Many of the most easily understood drug interactions are mediated by drug-induced changes in drug absorption, distribution, metabolism, and excretion. Certain drug interactions are of great clinical importance, usually involving a drug with a narrow therapeutic window.

The induction and inhibition of drug metabolic enzymes are two common types of metabolic drug-drug interactions. Decreased or increased drug exposures can be caused by these interactions. For example, cytochrome P450 (CYP450) induction may reduce the plasma levels of co-administered drugs, resulting in increased formation of toxic metabolite or decreased efficacy. Conversely, CYP450 inhibition may increase the plasma levels of drugs leading to toxicity. In addition, significant changes in total or local drug exposures are also caused by inhibition of drug transporters due to a new chemical entity of co-medicated drugs.
